Perhaps this is well known but I've been away from the IPO scene for a while.
If a user attempts to transfer a call, gets voicemail, drops the enquiry call and then makes another enquiry call to the same extension, the transfer will not complete.
Basically, a secretary tries to transfer a call to her boss, gets VM and then tries again. This time the boss answers but when the secretary presses transfer nothing happens.
I've confirmed that I get the same problem here on our test system. This is on 4.1 I think but I need to check.
