Transfer incoming call to extension with CF to outside number

[Cambrex]

Problem: Incoming call is transferred to station with call-forwarding to an outside number. Transferred caller hears dead air while phone that is forwarded keeps ringing and the call never goes through. An outside caller can dial the call-forwarded number directly and the call will forward to the outside number.
Software version: Definity. G3siV12. R012.
system features: trunk-to-trunk transfer is 'restrict'
cos for all stations: trunk-to-trunk override = y and restrict CF off-net = n
system parameters call coverage/call forwarding
Forwarding: Call Forward Override = n
