Toshiba Satellite no longer recognizes DVD drive

I have a several year old Toshiba Satellite, a 3-spindle model (which is what I prefer for travel and general use). Has Windows XP Prof. with SP2 and all (most) patches since.

The system no longer recognizes the DVD drive -- except occasionally.

How, to begin with, do I test whether it is a software problem, or a hardware problem?
 
If you have, or can get someone to make for you, a Windows 98 floppy disk with CD support, boot to that. If the CD/DVD is seen, (probably as drive E) then you most likely have a software issue which you may be able to fix by removing the drivers in device manager and allowing windows to re-discover the device.
If on the other hand it doesn't see the device, you probably have a hardware issue. It could be the drive itself. Or, it could just be not quite connecting properly. It will be tricky, but you should be able to open the case and re-seat the cables connecting the drive to the Mobo.

The drive may be dirty and require cleaning.
Will it read a regular,clean,unscratched CD?
