Tool to create line chart

I would like to be able to create a line chart using the output of another command.

The output will be in the format of:

server1
XXX
server2
XYZ
server3
XAX

Where XXX, XAX, and XYZ are numbers.

Does anyone know of an easy way to do this?

Any help would be greatly appreciated.
