Tomcat 1.4.18 install w / IIS 5.0

I cannot seem to get to the examples page.

My environment:
NT2000
IIS5.0
Apache 2.0
Tomcat 1.4.18

IIS and Tomcat run fine as stand alones.

This version of Tomcat calls for the use of isapi_redirector2.dll and workers2.properties files. No uriworkermap.property file is required according to the following doc:
_{jakarta.apache.org/tomcat/tomcat-4.1-doc/jk2/jk2/installhowto,html}

Has anyone attempted to install under these parameters with this version?

Here is a printout of my log file:

The last two lines are no doubt the source of the problem but I'm not sure what the solution is.

I won't continue to add lenghty files to this post at this time however, if you have had this problem and know what files you need to see I will imediately post them per your request.
