Toll Fraud on a Norstar System

We are having Toll Fraud and cant seem find out how its being done we have turned off Through dial and there is no Disa dial allowed ,what other thing do I need to check , thanks in advance

What type of system and software level?

Most toll fraud is done through the voice mail.

don't forget to restrict 101 (pick codes) on your system and also with the carrier if you don't use them.
