Intermittently, we are experiencing the following errors which are causing our database to failover (Failsafe is operating on NT)

[Errors]
TNS-12500: TNS:listener failed to start a dedicated server process
TNS-12540: TNS:internal limit restriction exceeded
TNS-12560: TNS:protocol adapter error
TNS-00510: Internal limit restriction exceeded
32-bit Windows Error: 8: Exec format error.

It appears to be some sort of resource issue. Has any one had a similiar issue or can shed some light on this?

Here's what I got from Oracle.

Intermittent TNS-12500 Errors on Microsoft Windows NT
---------------------------------------------------------

1. Increase the amount of memory available to the system
either by removing unnecessary processes or increasing
the amount of physical memory.

2. Increasing the amount of swap space will also
temporarily help relieve the symptoms.

3. Disable OTRACE. OTRACE is a tracing feature that can
cause many problems. OTRACE is enabled by default.
To disable OTRACE:

a. Stop the Oracle database.
b. Go to the orant/otrace/admin directory.
c. Delete all files with a ‘.dat’ extension.
d. Restart the Oracle database.

4. The TNS-12500 can be a result of the 'processes'
parameter in the init<sid>.ora file being too low.

My guess is that it's number 4 - too low a value for the "processes" parameter.

Do you use OPS?
There are some other issues in it due to the need in additional locks, e.g. _lm_procs. If you're really on OPS try to use MTS, it saves resources GREATELY. In any case utilizing MTS may help to solve your problem even if you're not on OPS.
