TN568 (V4 Audix) octopus cable pinout needed

We just finished moving our office and discovered at some point during our move our TN568 octopus cable fell off of our G3SI cabinets. Does anyone have the pinout for this cable? Ive searched high & low all over avaya's site for this info and its not in any of the books for the V4 audix on their site.
