TM 3.1 Error when trying to create a set

Hi

I am trying to create a 500 set on one of our sites using TM3.1. The set validates ok but when I click on Finish it comes up with the following message:

ERROR: duplicate key violates unique constraint "telephonefield_pkey"

Has anyone had this and how can it be resolved.

TN and location fields need to be Unique to each set.
