Tie Line Loop

We have a T1 that we use for outgoing long distance. Occasionally when I look at the channels, I'll see two channels "looped" together, like this:

If I disable, then re-enable either channel it will disconnect this "loop", and all is fine again, until the next time this happens.

Any ideas on how this could be happening? If I don't keep an eye on this, resetting channels when needed, I've seen up to 10 channels of the T1 have these pair loops.

usually happens when a conf call doesn't rls the trunks, make sure the cls THFD HKD, might help.. also check that dialed dn to be sure it's not being used a cfxa target.. if i forward my phone to 9 or 9 plus, i will end up talking trunk to trunk
