Three Stacked 4228G Problem

We have three 4288G switches that are stacked using the Up/Down ports (24&25). All has been well for several years, except this week when one of the switch has become unstacked...several times.

On the switch, there is a unit LED (1-4)that indicates its placement in the stack. Twice this week, we had users complaining about connection problems and instead of the switches showing the Unit LED 1,2,3 they were showing 1,2,1.

Any suggestions on what could be causing this..
