I thought I would add more to this thread to help out anyone else. I was doing a migration of a Exchange 2000 server, and needed to exmerge/export some old accounts before I delete them.
You can use this procedure to revive a disabled account:
1. Reenable the account.
2. In the AD MMC, right click the account, choose Exchange Tasks, and pick "remove exchange attributes". (Don't fret.. the account will not be deleted).
3. Wait for / or force an AD replication cycle.
4. In Exchange System manager, open your "administrative group", right click "mailboxes" and pick "Run cleanup agent". Wait 2 minutes.
5. Refresh the display, the account in question should have a red X next to it. Right click the account and pick reconnect. Select the original active directory account.
You should now have access to the account.
