There´s a utility called erasedisk.bin it erases the complete flash memory, you use it the same way as if you were upgrading the OS on the PIX. After you erase the flash contents you have to load the OS from monitor mode. I would give that a try and if it does not solve the issue then it may be your flash. Just make sure you save your activation key and your configuration because it erases EVERYTHING. Unfortunately I lost the utility but if anyone out there has it I would also like to get hold of it.
