The form you selected could not be displayed

I'm trying to design an custom form in Outlook that users can fill in and return (a mini survey). I've got the form set up and tested in my personal forms folder and it works fine. When I move it to the organizational forms folder and get users to test it, they receive the form and can fill it out, but then they try to send it back, the error message comes up "The form you selected could not be displayed"

We are using Office XP on XP Pro with exchange 2000 servers.
