I am trying to create a survey template.

I would like questions to be locked so people can not edit the questions.

I would like an answer box after the question where they would be able to type in their answer.

I have seen this done before but i have no idea how they did this...any help would be greatful
 
What product? If you are talking about Word 2003, have a look at the Forms toolbar.

As Lilliabeth says, you need to write the questions and insert Text Form Fields for the answers. (Note that you can limit the number of characters to stop the fields growing too big.)

When you have completed the form, if you now 'Protect as a Form', only the Fields will be editable.

If you are indeed asking about Word - and BTW, it is a good idea when posting to actually mention what application you are asking about - then, type "forms" into Word help. Then select "Create forms that users complete in Word".

It is a fairly complete Help topic.

Help can be quite useful when you use it.
