Teleworker Hot desk can't dial out after logging in

Ok I have a strange one. I have a teleworker that is a hotdesk user and all was working fine until this morning. when she logs into the phone she is unable to make outbound calls. She gets a busy signal. She is able to receive calls. I have logged her in on a phone in our office and have the same problem. I have deleted her account and created a new with the same number. This is happening just to this one user. We are running a a MXE with 9.0.3.29 ( we are upgrading to 5.0 in the near future) Any ideas?
 
outbound as in out of the system or including internal? does anything display on the set screen when busy signal is heard?
 
We think we have found the issue just surprised nobody else has complained. For some reason the CPN substitution is causing the problem. This site is in Germany and have no idea what could of changed with the Telco provider. We have not had any updates from them. When we removed the CPN substitution and just use the default the calls are going out but now display wrong CID. I will make a phone call to the telco provider to see if there has been changes made.
