TDM problem on IP406 3.0DT

hi
i have issue with users on 2nd expansion port being cut-off (DT16)
i have swapped the DT16 module but problem remains. the system itself does not reboot but seems to be problem with TDM interconnect.i have run monitor and can see the following (below) at the time it occurs, any ideas?????

i assume you have swapped in a known working phone.

you will need to give much more info. start with system type, module types, releases of each, symptoms of the phone in questions, type of phone model number, etc.

 
Defective extension, shortcut in extension cabling, bad interconnect cable, bad socket on expansion module or base unit, bad power supply, bad firmware, bad base unit

These are the most common issues of this problem, if replacing the expansion unit does not help then replace cable and power supply, if the problem persists then replace IP Office base unit.
 
when testing always plug the phone in at the port on the system.

 
swapped psu swapped IP406v1 3.0.577 to IP403 3.0.577 1x DT16 exp port 1 1x DT16 exp port 2 1x Ph8 exp port3 all 5.0.577. swapped TDM interconnect cables. swapped expansion unit. all extensions on expansion module are cut off system does not reboot.??????????????????????????????????????????????????????????????
 
I assume you used the supplied cable between expansion module and base module.
