I have a CICS, 6.1 software, in my lab with expansion card and Flash 4 voicemail running just fine on 2 digit extensions, so no you will not be forced to a 3 digit dial plan.
I will editorialize and suggest you carefully select your expansion card as there are several flavors of card that fit that slot, but some are for fiber clocking and offer no extra extensions, and in my experience unless you buy from a legitimate reseller, you will find a lot of these cards incorrectly labelled.
There are also combo cards which give you extra extensions with other features. Also be ready to deal with default extension ranges which may not be sequential to your first 16. It does take a bit of programming to get it just they way you will likely want it for your 2 digit system.
FYI it might be a good time to change them to 3 digits anyway, you will open up the Hunt Groups programming section.

Yes 3 digits has its advantages but then I've got to redo the whole voicemail which will be hard to explain, get the right people to do the greetings etc etc...
