tape restore error

when i do a restore i get the this error
tar: 0511-188 Cannot create ./SDFURN/BTA_200105/xlog001: There is an input or ou
tput error.
to do my restore i am just using
tar -xvf /dev/rmt1 ./SDFURN
I did cleaning of the drive and when i do i rm -R * in the dir i get and error
rm: 0653-609 Cannot remove AP.TRANS.TYPE.
There is an input or output error. if any one has any ideals that would be great

My first guess is, you have a bad disk or a corrupted filesystem...

check out the command

errpt|grep disk

look like i dropped a disks have ibm service bringing out new disk
