Tape Identifies as 55555

We are having a problem with B.E. 8.6 version 3808 and an old DEC library combo that will occasionally inventory a tape 55555. Once it does this it will not re-inventory or label the tape correctly without a server reboot. We have moved it to a different server, different SCSI card yet get the same results. Anyone out there see anything like this?
