Table Of Contents doing odd things with page number etc

Hi folks,
I have a situation with Indesign CS3 that I'd be very grateful to receive some help with.

I have built a Table Of Contents and InD is doing something odd that I've not been able to remedy. This is an InD book with one file per chapter.

Where the chapter number is double digits (Chapter 10 and above) the TOC places the second instance of the page number immediately after the chapter name and then again in the place where is should be. Like this:

10. Chapter Name194 194

The chapters with single digit numbers do not have this issue. They display like:
9. Chapter Name 194

I have three levels to the TOC.
----------------
Part Number (no page numbers)
Chapter Names (with page numbers)
Headings (with page numbers)
----------------
The data is called from a hidden layer on the first page of each chapter and each Part No page.

I have no idea where this second instance of the page number is being generated from or why. There is also another issue.

Here is an example of the output:

8. Chapter eight title example 175
9. Chapter nine title example 192
10.Chapter ten title example202 202
11.Chapter eleven title example226 226
12.Chapter twelve title example238 238
13.Chapter Thirteen title example256 256
14.Chapter fourteen title example268 268

As you can, there are two issues. As soon as it hits double digit chapter numbers:
1) The tab between the chapter # and the chapter name does not work and must be added in manually on the TOC
2) As mentioned above, the page number starts to get tacked onto the end of the chapter title, along with being in its correct spot too.

Any ideas why this might occur would be greatly appreciated.
