Table headers are not repeating on every page

I have a report that prints multiple invoices. For invoices that span multiple pages, I want the table header to repeat on each page (includes address, invoice #, etc).

If the last table footer is the only row left to print, the page breaks to the next page, but the table header does not print and only that footer row does.

All other instances the table header is repeated.

I am not sure what is causing this, any ideas?
