T1 Wall Jack information

I'm looking at the T1 jack which was installed... There is some writing on a Pacific Bell sticker... can someone tell me what these #'s are referring to?

This is the normal information left by the Telco installer.
The last 6 digits 481008 is the required circuit id needed,
when working on the span.
