T1 Out of frame

I Have a PRI on a legend R7V14.2 That shows OOF between the 100D-U and the CSU. It seems to show up mostly when Dialing. I have the clock/Sync source as Slot 10 (which is where the 100D-U card Resides

Any Ideas?

Synch source to be loop probably, TTR availability?, are particular channels coincident with problem?, CSU has network loop source as well?, ..... bad card??
