System does not stop ringing

I have a customer who has a NEC ipk elite system, if someone calls in and the call does not get answered it will keep ringing on the receptionist phone after the person on the other end hangs up. When another person calls the same thing happens until all the lines are busy and no one can call in or out. Also, when one particular extension is used for an outgoing call, when the caller hangs up it will ring back to that extension until you pick it up.

 
Sounds like a trunk issue.
What sort of service do you have? Loop, ground start, T1, or ISDN PRI or BRI?
If this is analog service (loop or ground) have you tried breaking the service connection while this ringing is in progress? If it stops you're looking at an issue with the Telco. If it continues ringing there's something going on with the IPK.
 
It actually sounds like a disconnect supervision issue to me. You "might" try swapping tip/ring on one or two CO lines to see if it helps, also you can try to adjust the Disconnect Signal timer in 3-40. Are these Ground start or loop start trunks?
 
Whoops, yeah, what pave35 said, my answer is if it was a copper trunk.
