I'm not a telephone expert with our system, but know just enough to be mildly dangerous.

We have a NEC SV9100 VoIP system with SIP trunking through our ISP. We occasionally have someone call our number and try to enter an extension and they get an "Invalid Entry" message, and they keep doing it, and eventually it sends them to the operator. In my testing, I found that if I call in on my cell phone and its on speaker phone, that's when I get the "Invalid Entry" prompt. If it's not on speaker phone, it works fine.

Any ideas on what I could check?

I presume you already did this, but NEC reccomends settings 84-34 VoiPDB DTMF setup 01 relay mode to RFC2833. For me DTMF doesn't work at all if I don't set this. You can ask your SIP provider if they support / prefer H.245. Try playing with 84-34 through 84-39, and anything else that mentions DTMF.
