Supervised Transfer Problem

We did an upgrade from 4.0.10 to 4.1.12. Since then, we're having intermittant problems on some supervised transfers. When the operator presses the the final transfer button (after speaking with person ready to receive the call), both the caller and the recipient get put on hold (listening to hold music). The call has to be retrieved again and re-transferred. This seems to happen on about 5% of the calls, and we cannot seem to find anything in common with those calls vs. the ones that work. The only thing that has changed was the upgrade. Any clues?

You should run a monitor trace and see what happens when the transfer goes wrong.
That should be able to give you some clue.
