Suggestions for Linux Partitions/Names/Sizes

I have used preloaded linux boxes and I am now trying to build my own boxes for various purposes.

The one thing I can't seem to find in any of my books is a good description / suggestion for partition setup.

What is the purpose for the different partitions and what is a good rule of thumb for partition sizes and mount points?

What is the minimum partitions I need? I know i need a boot and a swap partition. I don't know about /, /usr, /var, /home?

I'm not really looking for specifics, I just want to understand the partition and the reasons behind setting them up.
