Ouch, I don't believe it's possible to recover a form from Access. Which is why the "Are you sure" box pops up.
Are you absolutely positive there is no backup. Another copy of the database tucked away somewhere? If the database is on your company's network, you could talk to your LAN guys. They may have nightly backups that you could recover the form from. Maq B-)

There *is* a way to undelete a table, so I think something similar may be available for a form. The one caviat, and I'm guessing it's too late--the table undelete is only available if you *DONT* close the .mdb file. So, if you say 'ooops', just leave the db open--for days if you have to. I'm posting this now without the solution (yet) just on the chance the mdb might still be open.
I'll look for that undelete method, or anyone else here who knows it go ahead and post it (it's on this forum in a faq or old message somewhere, and I have it copied at home, but i'm at work now).
