Structure of LDAP Login Query

Does anyone know the correct structure and syntax for an LDAP query for authentication?

I found the problem. It wasn'ta problem with the LDAP query. It was actually the certificate authentication that is required for the computers on our network. It was set up by a previous admin and I wasn't aware that the system was in place.
