Strange problems with user accounts

I have a user who starting having a strange problem with their phone today. The screen said requesting service even after a reboot. The phone showed to be in out of service-operational status in quick look.

So, I tried to give her a new phone. When I moved her to the new phone the screen just says unavailble. I then moved another user to the phone to make sure it wasn't a phone issue. The other user account worked fine. Seems to be just this account.

We deleted and rebuilt her account, same results. Phone just says unavailble. I built her a new account with a new extension for now. I used the translation table to route her old number to the new one until I figure this out. Another strange thing is we tried to build a hunt group to route the internal calls from old number to the new and it just sends everything straight to voicemail, but I don't know for which extension since it doesn't announce the number. We tried deleting the hunt group, but the calls for the old number still go straight to voicemail instead of the default route.

Server has been rebooted. Services are all up. Lost at this point??
 
I rebooted the SG120 that this phone was registered to and it resolved the issue. It also resolved the issue with the softphones not working. This switch was also one of my backup config switches so it appears something got out of sorts there. If I get any answers out of Shoretel TAC I will post that as well.
 
TAC could not figure this out. I did a burnflash on the switch and it went away.
