Strange Error in Session Manager 1

We encountered a problem in a session manager where incoming calls would not successfully go though. In the sip traces we found a strange error "500 Server Internal Error java.lang.RuntimeException: Exception in adaptation <adaptation name>". Anyone encountered this before and has resolved it?

need more information call from what to what ?? CM>SM etc etc , what do you see in your alarms , what happens if you run maintenance tests in SMGR

This couls also be a sip monitoring entity error blip.
