STOP: 0x0000007B: Inaccessible Boot Device

I am trying to install Windows 2000 server to an IBM PC Server 315. The problem I am having is that once the installation process re-boots for the first time and gets half way through the 2000 server splash screen I get a BSOD:
STOP: 0x0000007b (0xF741B84C,0xC0000034,0x00000000,0x00000000)
INACCESSABLE_BOOT_DEVICE.

I have upgraded the firmware on the SCSI card, and tried the most up to date w2k and NT 4.0 SCSI adapter drivers with no success. I have also successfully installed Windows 98SE, Windows ME, and Windows NT Server without a problem and attempted to upgrade from each of these O/S's without success. I have performed low level formats on the hard disk and numerous fdisk's, formats, fixmbr's, etc, etc. which leads me to believe that this is not a hardware issue - but I could be wrong.

Using Bart PE I can boot the system from CD and look at the registry. It's strange that I know through the SCSI BIOS that its IRQ is 11, but looking at it through the registry it says that it's set to 4. When I try to change it though it doesn't work - it just reverts back to its original value.

I know the hardware is old but the Adaptec 2940 SCSI adapter and the two IBM SCSI drives are both on the HCL for W2K Server.

Any help would be greatly appreciated.
