SRG - Local mode and normal mode sets

What type of sets, what firmware is on the sets, and what release/patch level is the SRG?
I've seen the issue with some older phase 1 phones on 1.0 SRG 50s and SRG 1.0 200/400 systems, but haven't personally experienced it on release 2 or release 3 systems with phase 2 or 11XX phones.

1120 & 1140s. SRG200 1.5. Customer says it happened on one of the SRG50s which was originally a 1.0 but was upgraded to 3.0 several months ago. I'll look at upgrading the patches although I didn't see anything in the readme files regarding this issue.
