SQL 2005 - Missing Indexes

Hi, just found this query online to show missing indexes and am a little confused. Based on the attached results, how would I go about setting up the missing indexes? I'm confused as to how to know what equality fields go together when creating index and also what includes go with what equality fields? Thanks, Joe
