Do you experience that.. the message has scored enough score... e.g. 7/1 10/1 and I have set rewrite_subject 1
but there is still no [SPAM] in spam mail. Why?

Have you restarted 'spamd' after changing settings?
Are you running more than one mail server, i.e is this happening on a gateway that is being bypassed by spammer choosing other MX records?

has restart many times and I have only 1 mail server with only 1 mx record...

I found it has log in maillog, with over-score noticed

Do you have both of these lines uncommented in your /etc/mail/spamassassin/local.cf file?

rewrite_subject 1
subject_tag [SPAM]

???

Surfinbox.com Business Internet Services - National Dialup, DSL, T-1 and more.

required_hits 10
rewrite_subject 1
subject_tag [SPAM]
report_header 1
...

strange it work before...

should there be any permission problem? what part should i alert?

Have you reinstalled SpamAssassin recently?
It appears to me that SpamAssassin 'spamd' is not reading the local.cf that you think it is.

It is possible that you have another local.cf on your system.
It is possible that spamd is not finding the local.cf that you DO have an is therefore running with some basic defaults.

try (as root) 'cd /; find -iname local.cf' and see if more instances of the file exist than what you expected.

finally.. I install again with latest spamd and qmail scanner and it work fine...

but it show also the level and report in full header view...

how to ignore this information for the user?
