Spam now going to internal public folders

Hi, this seems like we may be infected with something. We are now experiencing spam going to some of our public folders where nobody externally should or would know these email addresses. It seems that maybe somehow we may be infected with something that will receive spam into any email address on our system. Does this sound possibly? It's like no matter what the email addresses is, spam finds it. We run Trend Micro Office Scan and ScanMail for Exchange.
