Sort of (2nd Request) - how to send file to client?

I've asked this before without any real workable resolutions. Perhaps someone can simply provide an example for all to learn from.

One of our web apps allows the user to elect to have data downloaded which is to be saved on the user's disk. Previously we accomplished this with the proper mime type header preceeding the data. This caused the user's browser to popup a file save dialog box and because we output a filename in the mime header (ex: c:\abc.123) the dialog box would be pre-filled with this information and it worked quite well. Now, for some reason (I think newer browsers and newer OS's (XP) this no longer work the same. The popup dialog still appears but the prefilled filename is equal to the name of the script generating the html output. And the path is somewhere in the bowels of the windows system (I think temporary internet files area).
The users who use this regulary want it to work like it did (i.e.: pre-filled in with the path and filename). Based on our examination of our output we cannot see any difference from what it has always been.

Therefore, can someone who has done this PLEASE post a code snippet showing exactly the lines used to output the mime header lines which will absolutely do what we've discussed for both Win98/me and XP? BTW - under Linux is works fine.
