Software to Route Serial data over LAN?

I have some old software that communicates over Serial Port or through a modem. Is there anything I can do to route the data being sent to the com port to a specific IP address on our intranet, and then have that comp route that data to its com port? Any software out there?
 
Check with a company called Subnet Solutions in Calgary. I saw a demo of a product in their 'Substation Suite' that as I recall did this on pc's running Windows (32 bit). Here's a link to their web site:
