Softphone to hardphone problems

In the office we have been testing the softphone over a VPN connection, with mixed results.
I have a limited knowledge of routers and vpn's. I understand the basics and am able to set up the router, connect to the internet set up a working vpn tunnel and do some of the basic settings.
We have sent a colleague home who was able to successfully connect to the router via the vpn and then call me from there softphone to my desktop softphone.
The problem occurs as soon as they try to dial my handset or make an outside call, the home user can hear me but I can not hear them.
The home user turned there firewall off, no change.
Can anyone point us in the right direction.

You probably should turn off direct media path (if it's on)
