Single message sent... two copies delivered?

We had a user this morning forward a single message to another user in our office. She only created it once, only had the user in the To box, and only once. She only clicked send once.
The end user got the message... and then 2 mins later got the message again.

I looked in the Exchange logs and it seems to think that two messages were created and sent at the exact same time, even though the user didn't do that.

I have had the user since test sending out mail to my account, and I only got a single message from her (the same message as before that she had sent).

I have never seen this before - I'm not terribly worried unless this starts happening frequently - any ideas on what caused it?
I'm inclined to think that it is early morning user error, but this user is a morning person, very alert, and she is good with computers and not the type to send two of the same message and then claim that she only sent one.

ahh, at least it is Friday.
 
Can you duplicate the issue? i.e. is it every time she sends to this one person or anyone that it occurs? Or did it happen once?

Seems to have only happened that one time.
Very odd.
That seems to indicate user error.

The message came into the user A, it had a PDF attachment. She forwarded it on to user B, and user B got two copies of it.

We have seen odd behavior in the past with PDFs in messages, but only in relation to printing (if there is a PDF message attached and you try to print the contents of the mail, but not the PDF, just the regular text... it won't let you - you have to copy and paste it out into another text application or another message and then it will print).

We are upgrading our Exchange server and our Outlook system all to 2k3 either later today or over the weekend - so perhaps that will fix all of this (and by "fix" I mean "give us a whole new set of problems").
 
I have the same issue, but mine is recurring. I have one user who is on Outlook 2003 and many of the mails he sends are received twice. I am on Exchange 5.5. The user though connects remotely from an internet account, in case that makes a difference.
Please let me know if you've found a solution (or direction) to solving this problem.
