Simple ASP to MySQL form sample sought

Hi

i have a simple task. I have an html form with user contact info (bane, email, and comments) that where once a submit button is clicked i would like two things to happen:

to have the data stored in a MySQL database, and to have the contents of the form sent to me via email.

i need to use ASP and MySQL. if there is a working exampple of something like this i'd appreciate a link

Have you started this on your own yet or were you expecting someone to do it for you? How about taking a stab at it yourself, and then once you get stuck post your code and we can tell you what to change.
