Simple ActveX component stopped working

Guys

My project was working fine until last week. All of a sudden the page where I have the activeX control (OCX) is not working. I have no idea why it stopped working. I created very simple OCX control and thats also not working for me.please see the PDF below and any help is really appreciated.
