Siemens 3000 T1 slips and errored seconds

We have a Hipath 3750 with a T1 connected to a Verizon router. Recently the site has been unable to fax. Verizon sees slips and errored seconds on the T. I remotely reset the T in the PBX, Verizon reset their side, no change. We are clocking off that T. I saw a couple minor differences that I applied when comparing to another 3000 with no trouble in Lines/Networking - Routing Parameters but as expected no difference. I'll see if the local tech can reset the connection to Verizon on both ends. maybe reset the T1 card? I also suggested a loopback plug toward Verizon and have them take a look at the T status. Any other suggestions what could cause the T1 errors?
 
Has anyone tried swapping out the smart jack with a spare?
 
The T1 itself is clean. The 3000 connects to Verizon's equipment that takes the T1 and distributes it to the routers and provides the T1 to the PBX.
 
I had the tech make a T1 loopback plug, disconnect the cable from behind the 3000 and put a loop toward Verizon. The T then ran clear, no errors, indicating the trouble definitely was the PBX. Reseating the T1 card did not help, so I had him power reset system. Then the T ran clear, no errors and a test fax was successful.
