Show only records who are ACtive but term date is not null 1

I must not be thinking clearly this morning, but I need to show records who's active box is checked, but the term date is not null.

Basically, in the table, I have a field that has an active check box, 1.00 or 0.00 and then a termination date field.

The report should show records that are active (1.00) but have a termination date.

The report is grouped on site, then provider.

Any help would be greatly appreciated.
