Sharing a Customized Form in Public Folders/Team Folders

I created a custom form to post documents to a discussion board in our team folder. However, once another team member opens the post and the document form, other team members are not able to use the custom form and Outlook displays a message,

"Custom Form cannot be viewed. The default Outlook form will be used instead."

I want the customized form to post documents on a discussion board in that folder only. How can I fix this problem?

I don't know if this is relevant but it might be if you or some of you are using Outlook 2000.

We had a similar problem - running Outlook 98 on our standard platform, but with staff dialling in from their home pcs with Outlook 2000 installed and accessing some custom forms.

When users with Microsoft Outlook 2000 accessed one of our custom forms - that form became "one-off". Apparently these one-off forms were being generated because Microsoft changed the way the VB script code running the form operated.

Apart from changing the script code, we also had to install a patch to all pcs running Outlook 2000 that were accessing the custom forms. This patch can only be applied to a client installation that is at Service Release 1 (SR1).

The patch is referenced:
Office 2000 Hotfix;OUTLOOK;4524;ENG

Anyway, probably doesn't help - but just in case.
