If am not wrong, it parses through the registry and counts # of apps using the DLL hence incrementing the count. The option to leave during unistall is to make sure that you do not break other apps that are using the same dll, eg if you were to uninstall wsock32.dll with your app, it would leave all your apps that use communications broken.
