Several entries to the same form 1

I am trying to create a sub table that collects several discreet information points but linked to the main table. I get an error message that it can't be done because it would create duplicates.
This is the scenario - Main Table has survey #10
Sub Table links to survey #10 and collects a list of name in a continuous form because they are contacts for survey #10.

Any ideas on how I might resolve this.

A classic one to many relationship between tables. Primary Key of "Main Table" is joinned with the Foreign Key of "Sub Table" obviously on SurveyID. SurveyID should NOT be primary key and NOT indexed to disallow duplicates for "Sub Table". That field along with a second field might create a Primary Key on the "Sub Table".
