If I have a web server connected to a phone line and running a fully operational web server, how can I let it be accessed on the internet. Do I have to buy a domain name, and connected through a ISP? Or can I just run it straight off (assuming all the settings on my server are correct).
 
You can access it via the IP address, but a domain name makes it easier for the customer out there.

Your problem will be that normally your IP address allocated on a dialup is dynamic (Server DHCP allocated) and will theoretically never be the same between dialups, this will make it virtually impossible for someone to find your site out there again.

To register a domain you will need to have a static IP, and unless you can arrange something with your ISP you are basically screwed. But I believe there is some software out there which will link a dynamic IP to your domain name, you will have to do some "surfing" at Tucows etc. IBM Certified Specialist - MQSeries
 
I have Road Runner at home (Cable modem) which is dynamic ip. I use a service DTDNS. I have a script that runs when my firewall restarts (which is extremly rare) that updates DTDNS with my new IP. Example of a host name you could get is -- myhost.darktech.com or something to that effect.

Then customers could enter that for connection to your web server.
