server problem or error in document structure

Is this a problem for all your universe or just one. This is generally a problem when you haven't set up your server correctly. If you don't have you domains set up right or some other connectivity problem, this can happen.
However, since you have used the cadenza word, my guess is that you know what you're doing and this is not the case. Very few people know that cadenza was the internal code name for the WebI 6 query panel.
